    Product Hardware Serial No.* Your product's serial number can be found on a barcoded a sticker on the rear or bottom of the item.  It will begin with '(21)'.

    Which Software Version** We'll need to know exactly what software version number you have now.  Enter the complete software title (the name of the software), and the specific version number in this field.  The Software Version information is displayed on the CD sleeve included with your software, or on the splash screen which appears when the software is launched.
    Software Authorization Code*** Your Authorization Code can be found printed on the installation CD sleeve included with your product.

    **We'll need to know exactly what software version number you have now. Enter the complete software title (the name of the software), and the specific version number in this field. The Software Version information is displayed on the CD sleeve included with your software, or on the splash screen which appears when the software is launched.
    ***Your Authorization Code can be found printed on the installation CD sleeve included with your product. If you do not have an authorization code, enter 'Missing' into this field, and explain further in the 'Additional Notes' field above.
